[connectedkids] fax from CJ Marbley

I have a fax from CJ Marbley and I can't read the bottom where his/her phone/e-mail info must be.  I have had Paul sign it and fill in a couple of spots, but there's a spot asking for a "sales contact" and I'm not sure what to put.  If this is a person who would make decisions regarding the library spending dollars, it would probably be my boss (Paul) but if it's a COnnected Kids contact thing, it would probably be me ... and if it's a bookkeeping thing, it would be Tracy ... Can anybody tell me exactly what the form is for so I can figure out what to put there so I can send it back ... AND can somebody tell me WHERE to send it back to?  I have CJ Marbley at x6498 ... but that's it!  THANK YOU!  Kim McMann
